안근마비 없이 두통으로만 나타난 내직근 안와근염

33세 여자가 3일 전부터 시작된 우측 두통 및 안구 주위 통증으로 내원하였다. 통증은 우측 측두와 안구 주위의 쑤시는 듯한 통증으로, 우측 주시에 의해 악화되었다. 환자는 복시를 호소하지 않았으며 신경계진찰에서도 안구운동의 장애는 관찰되지 않았다(Fig. A). 갑상선호르몬, 면역글로불린 G4 (122 mg/dL)를 포함한 자가면역질환검사는 정상이었으며 뇌 자기공명영상 또한 정상이었다. 안와 자기공명영상의 T1강조영상에서 우측 내직근의 비후 및 조영증강이 관찰되어(Fig. B) 우측 내직근의 안와근염으로 진단하였다. 경구 스테로이드 투여를 시작하였고, 이후 두통 및 안구주위 통증은 점차 호전되었다. 안와근염은 주로 단일 외안근을 침범하는 염증성 질환으로, 통증을 동반한 안구운동장애와 복시를 보이는 것이 특징이다[1,2]. 저자들은 안구운동장애 없이 두통만을 호소한 환자가 안와근염으로 진단된 증례를 보고하는 바이다.
