Diffusion-Weighted MRI Findings of Ischemic Optic Neuropathy

37세 여성이 미용 목적으로 눈 주위와 코 옆에 자가 지방을 주입한 직후 발생한 좌안의 시력감소와 언어장애로 병원에 왔다. 확산강조자기공명영상에서 왼쪽 망막과 시신경에 허혈시신경병 소견을 나타내었으며(Fig. A, B), 왼쪽 뇌반구에 다발성 뇌경색을 나타내었다(Fig. C). 안저검사에서는 망막 허혈 소견과 지방색전증이 관찰되었다(Fig. D). 지방색전이 안동맥을 통해 혈류에 역행하여 망막중심동맥, 경동맥을 통해 다발성 폐색을 일으켰을 것으로 생각되었다.

시신경의 급성 경색의 확산강조영상 소견은 뇌경색과 같은 영상 소견을 기대할 수 있지만[1], 시신경에서는 뇌와는 달리 낮은 민감도와 높은 특이도를 보인다[2]. 이러한 높은 거짓음성률은 시신경이 얇기 때문에 주변 조직의 자화율인공물(susceptibility artifact)과 부분용적평균화효과(partial volume averaging effect) 때문인 것으로 알려져 있다[2].
