Two Cases of Invasive Aspergillosis of Sino-nasal Origin

Abstract
Cerebral aspergillosis is a rare condition that affects primarily the immunocompromised host. Most of cerebral aspergillosis is developed by hematogenous dissemination from extracranial foci, but aspergillosis of sino-nasal origin rarely affects the CNS. In case 1, we describe a 78 year-old woman presented by intermittent fever and cough, and aki-netic mutism. PNS and brain CT scans showed filling mass in the PNS, extending intracranially with skull base ero-sion. In case 2, a 67 year-old woman who had taken steroid for presumed optic neuritis one month ago was presented with right ophthalmoplegia. Orbit MRI and PNS CT showed well enhanced lesions of right optic canal, cavernous sinus, and anterior aspect of middle cranial fossa with aggressive sinusitis. In both cases, the pathology of PNS showed septate hyphae with branching. They were treated with antifungal agent, with the resection of PNS in case 2, but were expired a month after admission.
