78세 여성이 패혈쇼크로 중환자실에 입원하였다. 급성신부전을 동반하여 우측 내경정맥에 비터널식 투석용 카테터를 삽입하고 혈액투석을 시행하였다. 입원 21일째에 호흡곤란으로 빈호흡이 있는 상태에서 바로 누운 자세에서 카테터를 제거하고 상처 위에 밀봉드레싱 요법을 하였다. 제거 직후 반혼수 상태로 의식저하가 있었으며 National Institutes of Health Stroke Scale 뇌졸중척도는 28점이었다. 뇌 computed tomography (CT)와 magnetic resonance imaging에서 주로 우측 대뇌에서 공기색전증이 관찰되었으며(Fig.), 감수성강조영상(susceptibility-weighted imaging)에서 CT보다 더 많은 수의 공기 음영이 관찰되었다(Fig. C). 심초음파검사상 우-좌 단락은 관찰되지 않았다. 카테터 제거 후 공기가 주입되어 발생한 역방향 대뇌정맥 공기색전증으로[1] 우측 내경정맥을 통하여 공기가 주입되어 주로 우측 대뇌에 공기색전증이 발생하였다. 환자는 활력징후가 불안정하여 고압산소 치료는 시행하지 못하였고 당일 사망하였다. 공기는 매우 낮은 자화율로 자기장에서 신호 소실을 유발하기 때문에[2] 감수성강조영상에서 공기색전이 저음영으로 잘 관찰된다.
