마르키아파바-비냐미병을 동반한 베르니케뇌병증

40세 여자가 전일 발생한 의식변화로 내원하였다. 20년 전부터 매일 소주 1병, 1년 전부터는 2병을 먹었고, 3개월 전부터 식사를 하지 않았다. 의식은 혼미하였고 수평안구운동이 불가능했고 사지와 체간에 실조증을 보였다. 티아민 투여 후 수시간 만에 의식과 외안근운동이 호전되었다. 액체감쇠역전회복영상에서 베르니케뇌병증에서 전형적으로 보이는 내측시상, 중뇌수도관주위회질에 대칭적으로 고신호강도병변이 보였다(Fig. A-C). 양측 기저핵병변 또한 드물게 베르니케뇌병증에서 보고되는 소견이다[1]. 뇌량 전체에 걸쳐 고신호강도가 보였는데(Fig. D-F) T2강조영상에서 뇌량의 전체 및 주변 백질에 고신호강도병변을 보이는 것이 마르키아파바-비냐미병에서 특징적인 영상 소견이다. 만성 알코올 남용과 특징적인 뇌 MRI를 근거로 마르키아파바-비냐미병을 진단하였다.

Bilateral posterior putamen lesions are rarely reported (B, C, arrows). In addition, FLAIR images reveal high signal intensity lesions on entire genu, body, and splenium of corpus callosum which is consistent with Marchiafava-bignami disease (C, D, E, F). 마르키아파바-비냐미병은 다양한 임상양상을 보이며 일부에서 혈중 티아민 농도가 감소되어 있어서 빠른 티아민 투여가 예후에 중요하다[2]. 영양부족, 티아민 결핍과 연관된 뇌병증인 베르니케뇌병증과 마르키아파바-비냐미병이 동시에 발생한 경우이다.
