일산화탄소 중독과 동반된 뇌경색과 출혈변환

Trans Abstract

Carbon monoxide poisoning causes hypoxic brain damage with various neurological complications. Cerebral hemorrhagic infarction has rarely been reported as a complication of carbon monoxide poisoning. The author experienced a patient who developed cerebral infarction in the striatocapsular area extending to the corona radiata after carbon monoxide poisoning, which was followed by hemorrhagic transformation associated with neurological deterioration.

일산화탄소는 독성이 강한 불완전 연소물질로 이 물질에 수시간 노출 시 일산화탄소중독이 발생할 수 있다. 일산화탄소중독은 일산화탄소가 산소대신 적혈구의 혈색소와 결합하여 일산화탄소혈색소(carboxyhemoglobin, COHb)를 형성하기 때문에 적혈구의 산소 결합을 방해하고 조직으로의 산소 전달을 감소시켜 저산소증을 일으킨다[1]. 그로 인하여 산소요구량이 많은 조직인 뇌와 심장이 주로 손상된다. 저산소증에 의한 뇌손상은 특징적으로 기저핵, 해마, 소뇌, 백색질 부위를 침범한다[2,3]. 일산화탄소 중독 후에 발생하는 이러한 저산소뇌손상 외에 드물게 뇌경색이 발생할 수 있는데 주로 출혈경색(hemorrhagic infarction)의 양상으로 나타난다[4-6]. 저자는 일산화탄소중독 후에 저산소뇌손상과 함께 뇌경색이 발생한 후 수일이 지나서 출혈변환(hemorrhagic transformation)이 나타난 예를 경험하여 이를 보고하는 바이다.

증 례

37세 남자가 의식이 저하된 상태로 응급실로 이송되었다. 환자는 전날 저녁까지는 건강한 상태였으나 내원일 오전 8시경 집에서 의식을 잃고 쓰러져 있는 것이 발견되었다. 방 안에는 연탄이 타고 남은 재가 있었고, 옆에서 같이 자던 모친은 이미 사망한 상태였다.

환자는 특이 과거병력은 없었다. 흡연은 하지 않았으며, 음주는 월 1-2회 소주 1병 정도 하였다. 내원 당시 활력징후는 혈압 115/70 mmHg, 맥박 84회/분, 체온 36.5°C였다. 의식은 혼미하였고, 글래스고혼수척도(Glasgow coma scale, GCS)는 E2M4V1이었다. 신경학적검사상 대광반사, 각막반사, 인형눈반사는 정상이었다. 통증 자극에 대한 반응은 좌측 상하지의 움직임이 MRC (Medical Research Council) grade 2로 저하되어 있었다. 건반사는 정상이었고 바빈스키징후는 나타나지 않았다. 혈액검사상 COHb가 5.3% (정상치: <0.8%) 로 상승되어 있었고, 혈중 트로포닌-T는 0.252 ng/mL (정상치: <0.1 ng/mL), 크레아틴인산화효소는 596 U/L (정상치: 26-174 U/L), 크레아틴인산화효소-MB는 29.34 ng/mL (정상치: <5 ng/mL)로 상승되어 있었다. 흉부 엑스선, 심전도, 흉부경유심초음파는 정상이었다. 식도경유심초음파와 홀터감시는 시행하지 못했으나 중환자실에 입원 중 시행한 심전도감시에서 특이소견은 없었다. 내원일 오후 5시에 촬영한 확산강조영상에서 양측 창백핵(globus pallidus)과 해마에 저산소뇌손상을 시사하는 고신호강도와 함께 우측 줄무늬체-내포와 대뇌부챗살에 급성뇌경색이 관찰되었고 출혈소견은 보이지 않았다(Fig. 1). MRA는 시행하지 못했으나 액체감쇠회복역전영상(fluid-attenuated inversion recovery imaging, FLAIR)에서 우측 중대뇌동맥의 신호강도는 정상적으로 보였다.

환자는 이후 중환자실에서 기도삽관 후 인공호흡기에 의존하면서 고압산소치료를 시행 받았다. 환자의 의식은 점차 호전되어서 졸리지만 간단한 명령수행이 가능할 정도가 되었으나 입원 7일째 의식이 혼미하게 저하되어서 CT (computerized tomography)와 MRI (magnetic resonance imaging)를 시행하였다. CT상 이전의 뇌경색 부위인 우측 기저핵과 대뇌부챗살 부위에 혈종과 주변부의 부종이 관찰되었고 덩이효과(mass effect)로 주변 뇌실이 밀리는 소견이 관찰되었다(Fig. 2). MRI에서 상기의 출혈병변은 T1강조 영상에서 동신호강도를 보이고 T2강조영상에서 중앙부의 고신호 강도와 주변부의 저신호강도를 보여서 급성기 혈종을 시사하였다. 이후 보존적인 치료를 받으며 환자상태는 다소 호전되었으나 GCS는 E4M4Vt로 의사소통은 되지 않았고 좌측 상하지에 심한 마비(MRC grade 1)를 가진 상태로 입원 40일째 퇴원하였다.

고 찰

본 증례는 뇌졸중 위험인자를 가지지 않은 젊은 남자에서 일산화탄소중독으로 인해서 저산소뇌손상과 함께 뇌경색이 발생했으며 수일이 지나서 신경학적악화를 동반한 출혈변환이 나타난 예이다.

일산화탄소중독에 의한 저산소뇌손상은 창백핵을 가장 흔하게 침범하며 해마, 소뇌, 백색질, 다른 기저핵 부위 등에도 나타날 수 있다[2,3]. 뇌손상의 부위와 범위는 일산화탄소에 노출된 시간과 정도에 따라서 다르다[2]. 병리적으로는 창백핵의 괴사와 점상출혈, 백색질의 탈수초, 대뇌피질의 해면성 변화, 소뇌의 조롱박세포(Purkinje cell)의 소실 등이 관찰된다[7]. 이러한 저산소뇌병변 외에 뇌경색이나 뇌출혈이 드물게 동반될 수 있는데 주로 발병 초기에 출혈경색의 양상으로 나타난다[4-6,8,9].

본 증례에서는 일산화탄소중독 후 24시간 이내에 촬영한 뇌영상에서 저산소뇌병변과 함께 뇌경색이 발견되었다. 뇌경색이 일산화탄소중독과 무관하게 우연히 발생했을 수도 있으나 뇌경색 위험 인자가 없었던 젊은 환자이고, 뇌경색 발생시기가 일산화탄소중독 직후인 점을 고려할 때 일산화탄소중독과 연관되어서 뇌경색이 발생했을 가능성이 높다. 환자는 뇌경색이 발생하고 수일이 지난 후 실질 내혈종 양상의 출혈변환에 의해서 신경학적 상태가 악화되었는데 이런 경우는 매우 드문 예이다. 이전까지 보고된 예에서는 모두 3일 이내에 촬영한 뇌영상에서 출혈경색의 양상으로만 발견되었고 뇌경색이 발생한 후 1주가 지나서 출혈변환이 나타난 예는 없었으며, 실질내혈종 양상의 큰 출혈에 의해서 신경학적악화가 나타난 예도 없었다.

일산화탄소중독 후에 출혈경색이 발생하는 기전은 아직 정확히 밝혀지지는 않았지만 저산소증과 염증 반응이 기여할 것으로 생각되고 있다. 일산화탄소는 혈소판의 헴단백질과 결합하여 혈소판-중성구 응집을 일으키고 중성구의 혈관벽 유착이 일어난다. 이 과정에서 중성구에서 배출되는 골수세포형과산화효소(myeloperoxidase)와 활성산소는 혈관에 산화 스트레스를 주어서 혈관내피의 손상을 일으킨다. 또한 저산소증에 의한 세포괴사 및 세포자멸사도 염증과 손상에 기여한다. 이러한 혈관내피의 손상 및 염증반응에 의해서 뇌경색이 발생하고 출혈변환도 생길 것이다[1]. 심장 손상으로 인한 심장성색전증이나 혈액학적 합병증도 또 다른 원인으로 고려할 수 있다[4].

본 증례의 또 한 가지 특이한 점은 뇌경색의 병변부위가 줄무늬체-내포 및 대뇌부챗살이라는 점이다. 이전에 보고된 뇌경색 부위는 주로 중대뇌동맥 영역의 피질부위를 포함하는 큰 영역이었다[4,5,8]. 그 외의 부위로는 양측으로 광범위하게 반난형백색질중심부(centrum semiovale)를 침범한 예가 있었다[6].

결론적으로 본 증례는 일산화탄소중독과 동반되어 뇌경색과 뇌출혈이 발생할 수 있음을 보여준다. 일산화탄소에 중독된 환자에서 국소신경학적결손이 관찰되면 뇌경색의 동반 가능성을 고려해야 하며, 뇌경색이 발생한 후에도 출혈변환에 의해서 환자 상태가 악화될 수 있음을 유의하고 치료해야 한다.
