무증상의 긴 분절대동맥박리에서 시작된 경동맥박리

59세 여자가 목통증과 좌측 편마비(Medical Research Council Grade III)가 관찰되었다. 뇌자기공명영상에서 우측 대뇌반구의 다발성뇌경색과 경동맥 주위의 내막하혈종이 초승달 모양으로 관찰되어 동맥박리에 의한 뇌경색을 시사하였다(Fig.). 동맥박리의 기시부를 확인하기 위해 시행한 조영증강뇌자기공명혈관조영술 및 대동맥혈관전산화단층촬영에서 경동맥부터 상행 및 하행 대동맥까지 침범된 긴 분절의 동맥박리를 확인하였다(Fig.). 응급으로 대동맥치환술(ascending and descending aortic replacement)을 시행하였고 신경계증상도 호전되었다.

대뇌동맥박리는 주로 경동맥이나 척추동맥에서 발생하나 대동맥에서 시작되어 발생할 수도 있다. 급성 대동맥박리는 주로 흉통이 동반되나 통증 없이 신경계증상만 초기에 나타나는 경우 진단이 지연될 수 있고[1,2], 이를 인지하지 못하고 혈전용해제 등의 치료를 할 경우 심장눌림증(cardiac tamponade)이나 종격출혈 등의 위험이 있어 박리의 기시부가 불분명하다면 반드시 대동맥을 포함한 정확한 평가가 필요하다.
